Annual Registration:Please submit one registration form form and one medical form form per child along with the $20.00 enrollment fee per family.

Please make checks payable to Studio 4 Dance and mail to: Studio 4 Dance, LLC, 1805 E. 121st St. Burnsville, MN 55337.

All fees are due to the studio by the 10th of each month (credit card payments not accepted). A $5.00 late fee will be assessed for payments received after the 10th of the month.

There will be a $25.00 fee assessed for returned checks.

A 5% discount for tuition paid-in-full by September 10, 2010 (9 month session).

Tuition is based on a 32-lesson year, not dependent on # of classes per month.

Family Plan: First child per family full price, each additional child receives $5.00 off of their monthly tuition.
